Technical Support will assist in diagnosing the reported problem and if product return is deemed necessary, Technical Support will provide a Returned Material Authorization (RMA) number. Product must be returned prepaid, with proof of purchase date, and a description of the problem to Leviton Manufacturing Co., Inc., Attn: Quality Assurance Department, 2222 - 222nd Street SE, Bothell, WA 98021-4442.
