The Leviton Remembrance CTP-8-1044 is a (DISCONTINUED) 4 slider + 1 master slide, + 4 presets + off preset wall-mount analog control station for theatrical and architectural lighting.
As part of the NSI/Leviton Remembrance series, it is a DMX-based lighting controller that manages up to 512 channels.
The CTP prefix indicates it was part of the Colortran-branded Remembrance line.
